If you wish to request a period of absence for your child, please use one of the forms available here.
- If the absence is for a short appointment (such as a medical/dental appointment, music exam or visit to other school) use the 'Term Time Appointment Request' form.
- If the absence is for an extended period of time (i.e. longer than a couple of hours) use the 'Leave of Absence Request' form.
